Your pet has to stay Unhappy long enough to regress to Loyalty level 1 and stay that way for a good long time.
To be quite honest, once you get to second or third level loyalty, you'd have to be trying in order to have your pet run away on you.-(Back in Retirement) Draka:

Judging from a lengthy E3 demo, BioWare's Star Wars MMO plays like a Knights of the Old Republic sequel.
